4.  The Transfer Request form will be displayed.  The "From" Warehouse will automatically fill with the Warehouse - this may be changed it this is not the correct Warehouse.

The remainder of the process is identical to creating a Transfer Request from the Inventory menu.  Follow the link to Page 2 of Transfer Requests from Inventory for instructions on filling in the Transfer Request form and the remainder of the process.
